CE Provider Contact Information Change

Use this form when there are any changes to the contact information for ASHA Approved CE Provider organizations, CE Administrators (CEA), or CE Content Consultants (CECC). To change the name of an ASHA Approved CE Provider's organization, please use the Changes to ASHA Approved Provider Organization Notification Form. To change the CEA or CECC, please use the Changing Continuing Education Administrator Form or the Changing Continuing Education Content Consultant Form.

All changes should be updated within 3–5 business days.
