Photo and Video Release

In order to further the work of the American Speech-Language-Hearing Association (ASHA), American Speech-Language-Hearing Foundation (ASHF), Special Interest Groups, and National Student Speech Language Hearing Association (NSSLHA) and for other good and legal considerations, receipt and sufficiency of which is hereby acknowledged, I do hereby consent, irrevocably, to the copyright use and/or publication by ASHA, ASHF, Special Interest Groups, and NSSLHA of the photograph(s) and all photographic, prints, stills, video, electronic media, or the reproductions of all or part of said photographs, prints, stills, motion pictures, electronic media, or reproductions.

And hereby consent further to the use of the name of the person who appears in the photographs, negatives, prints, stills, motion pictures, electronic media, or other reproductions in any way and at any time and from time to time in connection with any ASHA, ASHF, Special Interest Groups, or NSSLHA programs. The said photographs, negatives, prints, stills, motion pictures, electronic media, and reproductions are and shall remain the property of ASHA, ASHF, Special Interest Groups, and NSSLHA.
