Name Change Request
Thank you for submitting your name change request! Requests are generally processed within 1–2 business days. If you are currently affiliated with ASHA or NSSLHA, you will receive a new ASHA or NSSLHA ID card in the mail within 7–10 business days at your My ASHA Account primary mailing address.
Remember, if you have not already done so, to contact your state licensing board to update your name with them as well. You can find contact information for your state licensing board on our website by selecting your state and then selecting the link under the Contact Information header.
