Reporting by Excel files.
This option is open to all Providers. If you are interested in using Excel to report participants, go to the "Reporting and Roster Correction" section of the CE Provider Portal that can be found in the Navigation Bar at the top of each page, read these instructions and use this template [XLS] as an example.
Courses offerings held with no ASHA CEU participants.
Course offerings not held or cancelled.
This option is open to all Providers. Go to the "Registration and Cancellation" section of the CE Provider Portal.
This option is open only to Providers who have been approved to transmit participant records electronically by .csv file. If you are interested in using this process, first read the details and specifications [PDF] and then notify your organization's ASHA CE Accounts Manager of your interest.
This option is currently under development and will be available to Providers who have reporting systems that can communicate database to database. Please contact your ASHA CE Accounts Manager for more information.
This option is open to all ASHA Approved CE Providers.

This option is open to all Providers. ASHA CEU Participant Forms and the accompanying Course Offering Report Form can be faxed to this toll-free number 866-271-3040 only. No other items may be faxed to this number. Please read these instructions for reporting by fax.
Providers have several options for submitting reporting materials to ASHA CE. In addition to mailing the Course Offering Report Forms and ASHA CEU Participant Forms, providers may choose to report this information online or by fax, with certain restrictions.
