CE Provider Course Offering Reporting Form
As of August 26, 2020, the CE Provider Portal will be an option to submit zero participants reporting. Log into the system, go to “Reporting and Rosters” and then “Submit Offering Reporting.” Please view the Rosters section of the CE Provider Portal User Guide [PDF] for detailed instructions.
Use this form to:
- Use this form to report course offerings as held but where no participants requested ASHA CEUs
- To cancel a course offering, go to the Course and Offering Registration and Cancellation System, view the offering overview page, select the offering you intend to cancel, and then click on the cancel button.
