Register for NOMS in Schools

We hope that these materials have answered your questions about NOMS and that you are now interested in subscribing. To begin the registration process, you should complete the appropriate Subscriber Registration Form (School System or Individual Clinician) and sign two copies of the Letter of Agreement located below.

School System

Individual Clinician

You may use the same set of registration materials for both the Pre-Kindergarten and Kindergarten through Grade 12 components as long as the Subscriber is the same. If you choose to have a separate subscriber for each component, duplicate these registration forms and have each subscriber sign two copies of the Letters of Agreement. The Registration Form(s) and signed copies of the Letter of Agreement should be mailed to:

Once we receive your registration forms, we will send you the training materials. This self-study training program needs to be reviewed before completing the User Registration Test. If you are participating as part of a school system, ASHA will notify both the Subscriber and the clinician of the results of the test. As soon as you (as an individual participant) or any member of your staff (for system level participation) successfully completes this test, we will forward your forms for data collection if that is the method of data collection you indicated on the registration forms.
