ASHA State Grants: Budget Proposal Form
Please complete the funding proposal for your project for both expenses where ASHA funding will be used and expenses where state funds will be used. Please indicate not applicable with a zero.
Examples of in-kind resources including volunteering to attend meetings/hearings and utilizing existing state association paid staff.
- The amount of each grant will be determined at ASHA's sole discretion. A maximum of $12,000 per application may be awarded. Therefore, ASHA funds requested should not exceed this amount. Please check your totals. State associations are expected to contribute to the costs of the proposed project either financially or with in-kind services. ASHA's decisions on grant awards and amounts are final.
