Submission of Materials for Clinical Fellowship

As you begin your Clinical Fellowship, there is no paperwork that must be submitted to ASHA other than the application for certification.
At the conclusion of the experience, or if you change settings or supervisors or the number of hours you work per week, you must submit a Clinical Fellowship Report and Rating Form [PDF] to the ASHA National Office. You must complete a separate form for each supervisor or setting. Dates and information should only reflect the experience recorded on that form.
While a separate CF Report and Rating Form does not necessarily need to be submitted if you change from full-time to part-time experience, you must provide information on which weeks (dates) were part-time vs. full-time (i.e., 6/1/07 to 7/5/07 = 5 weeks at 22 hours per week; 7/9/07 to 7/23/07 = 2 weeks at 12 hours per week; 7/23/07 to 8/20/07 = 3 weeks at 36 hours per week; 8/20/07 to 9/17/07 = 4 weeks at 22 hours per week).
The total number of weeks and hours submitted must clearly show that the requirement of 36 weeks of full-time experience, totalling an accumulation of 1,260 total hours, has been fulfilled.
