Description: This is a review of research studies investigating the effect of cranio-maxillofacial osteotomies and distraction osteogeneis on the speech and velopharyngeal status. Although the treatment did not directly involve SLP services, it reports speech outcomes which may be helpful to clinicians treating individuals with craniofacial disorders.
Question(s) Addressed:
Question not specifically stated.
Population: Individuals with facial and cleft deformities.
Intervention/Assessment:
- Surgical techniques such as maxillary osteotomies, maxillary distraction, and palatal distraction.
- Methods for speech and velopharyngeal status assessment: perceptual speech analysis, cephalometric analysis, pressure-flow measurement, nasoendoscopy, videofluoroscopy, nasometer, nasal resistance, sonograph, lip pressure, tongue pressure, acoustic analysis, and machine analysis.
Number of Studies Included: 39
Years Included: 1971 - July 2004
Findings:
Conclusions:
- Treatment
- Speech
- Findings were mixed. Many studies found that surgery had no impact on speech and velopharyngeal status. Others reported deterioration of speech in patients with velopharyngeal impairment (VPI) or borderline VPI. No differences were noted between intervention type.
- No conclusions could be drawn due to methodological quality and variety of studies reviewed. Further research is warranted.
