The ASHA Leader Article Proposal Questionnaire

1. How should the author(s) name(s) appear in the byline?
(Please limit the number of co-authors of an article to no more than four. List the authors in the order that they should appear, beginning with the top box. Include each author's academic degree and ASHA certification, if certified.)

3. Please type a brief summary of the major ideas you plan to present in your article.

4. What basis will your article have in research or evidence-based practice? Tell us briefly about that aspect of your proposed article.

5. How will you approach your topic? Will there be case studies? Will the piece be a general overview of your topic or more specific in its focus?

6. Are there any cross-disciplinary aspects to your topic that you could include in the article or as a sidebar?
