Decision Making in Adult Dysphagia
Format(s): eWorkshop
CEUs: 0.3
3.0
It is easy for clinicians in busy medical environments to evaluate swallowing one time, and from one perspective. Patients with multidimensional problems require more than one approach to assessment, and usually more than one profession. But inter- and intraprofessional agreement on swallow physiology and management can be a daunting challenge.

2.0
Feeding and swallowing problems affect many students in schools. To identify, manage, and assist these students, school staff need systematic procedures and coordinated effort among dysphagia team members. This program presents strategies for developing process and procedures for school teams for evaluation, problem solving, the development and consistent use of feeding plans, and staff education and training. The presenters will also describe practical and effective research-based therapeutic and feeding strategies that work in school settings. Treatment strategies that foster improvement in self-feeding, oral-sensory and oral-motor skills, and swallowing are emphasized.

In the comprehensive evaluation of infants and children with swallowing and feeding problems, the importance of knowledge regarding underlying etiologies, status of nutrition and hydration, and upper airway/pulmonary status cannot be overstated. This video self-study presents “typical” development as a basis for evaluating oral skills and swallowing. Strategies for clinical observations and instrumental considerations are presented, and video clips of infants and children enhance the learning experience. This program covers infants including neonates and children up to age 10.

Evidence-Based Decisions for Tube Feeding in Clients With Advanced Dementia
Format(s): eWorkshop
CEUs: 0.1
1.0
For people with advanced dementia who are permanently recumbent, aspiration is a serious risk. Aspiration and malnutrition are common in these patients. Percutaneous endoscopic gastrostomies are commonly used to reduce the risk of aspiration pneumonia and to provide “adequate nutrition and hydration.” There are essentially no good randomized controlled trials of whether this intervention is effective for these indications. There is a priori little reason to believe that feeding via PEG-tube reduces the risks of aspiration pneumonia. The major adverse outcomes attributable to malnutrition are not improved in patients receiving tube feeding.

Managing Dysphagia in the Schools
Format(s): Journal (Online), Journal (Print)

The number of students with dysphagia has increased, augmented by growing numbers of graduates of neonatal intensive care units and children with autism spectrum disorder. Furthermore, changes in federal legislation and case law have clarified some of the issues that are central to administrative decisions, educational programs, and related services associated with dysphagia management in the schools. This growing population of children with special needs has stimulated increased interest and research. The contributing authors in this clinical forum, each with significant experience that informs their views and observations, report their research and provide reviews of the literature.

This issue of Perspectives discusses normal and disordered esophageal anatomy and physiology in adults and children. First, we review normal anatomy and physiologic function of the upper esophageal sphincter and esophagus in adults, children, and neonates. Then, we present the strengths and weaknesses of state-of-the-art diagnostic studies of the esophagus. In addition, we discuss the pathophysiology, clinical presentation, and current treatment options for eosinophilic esophagitis. Finally, we review guidelines for the performance of modified barium swallow studies and esophagrams and include a discussion of the rationale for SLPs to perform an esophageal screening.

Research confirms that neuroplasticity makes it possible to treat swallowing deficits behaviorally, even in persons with neurodegenerative disease. But treatment must be focused in order to yield positive or therapeutic results. This program reveals how to gain that focus. Review an array of strengthening and skill-building rehabilitation approaches and compensations that provide options for the majority of such patients. The goal is to equip you with concepts and tools to treat adults with neurodegenerative disease, and to advocate effectively on their behalf.

Using Measurement Tools in Assessment and Treatment of Dysphagia
Format(s): Journal (Online)
CEUs: 0.45
4.5
Accurate and consistent measurement during the assessment and treatment of feeding and swallowing disorders is necessary to ensure that clinical decisions best meet patient needs. This journal self-study presents five articles that describe recent research related to the measurement of eating and swallowing behaviors and tools. For those interested in pediatrics, this product addresses the development of a tool to more accurately assess pediatric behavioral feeding issues as well as a description of how chewing develops in young children. For those working with adults, this self-study includes a tutorial on the use of surface electromyography (sEMG) to assess and treat swallowing disorders as well as research on how the perceived taste of a bolus influences swallowing behaviors. A fifth article discusses how the line spread test can be used as a visual tool to quickly and more accurately determine liquid thickness.
