Adult Speech-Language Pathology Documentation Templates
ASHA's Ad Hoc Committee on Documentation (2007–2008)* developed documentation templates intended for use in electronic medical records. The information can be modified for paper format as well. These templates are consensus-based and do not represent the official policy of ASHA. ASHA members are not required to use these templates or the terminology included in them.

How to Use the Templates
You may use any or all of the information included in the templates. Feel free to modify or combine information as appropriate for your facility and patient population.
These templates are in PDF format. If you would like to receive the templates in Word, which can be more easily modified, please e-mail firstname.lastname@example.org. Please indicate which templates you would like to receive.
ASHA encourages the use of ASHA's National Outcomes Measurement System (NOMS) in these templates. Payers encourage reporting quality and outcomes measures, and NOMS has been specifically identified by the Centers for Medicare and Medicaid Services (CMS) as an approved measure. ASHA members can register free of charge to take a self-study program and become validated users of NOMS.
