Access documentation templates, which are intended for use in electronic medical records but can be modified for paper use. ASHA members are not required to use these templates and may modify or combine information as appropriate for their respective facilities and patient populations. Templates are currently available for use with adult and child populations.

Phonemic Inventories Across Languages
