Getting Started

How do I access the ASHA CE Electronic Course and Offering Registration Form?

The ASHA CE Electronic Course and Offering Registration Form is available online. Your login ID is your four-letter CE Provider code—for example, ABCD.

What types of courses can I register using the Electronic Course and Offering Registration Form?

  • Group Learning and Individual Learning Experiences
  • A course with one or more offerings
  • A cooperative offering for which you would make a payment online with a credit card or print a confirmation page and mail a check with the cooperative fee.

Note: The only type of course that you cannot register using the electronic form is an Independent Study.

How is the form organized?

The electronic form is organized in exactly the same way as the paper CE Course and Offering Registration Form. It contains the following key pieces of information:

  • Course Information and Description
  • Course continuing education units (CEUs)
  • Promotional Materials
  • Course Design
  • Planning and Instructional Personnel
  • Satisfactory Completion and Course Evaluation
  • Course Offerings
  • Attestations and Signature

How is the electronic form different from the paper version?

The electronic form allows the CE Administrator a third option, in addition to faxing and mailing, for submitting your course registration materials to ASHA CE. The electronic form asks for the same information in the same order as the paper Course and Offering Registration form. Plus the electronic form offers the following benefits:

  • You can enter and save a draft so you can edit it at your convenience.
  • You can create the course description in a Word document then cut and paste it into the form. Or you can type the course description directly into the form.
  • You can create the required attachments in Word or scan documents into PDF files and upload the files using the form. The form enables you to browse your own computer files to select and upload the required attachments.
  • You'll receive an e-mail confirmation when (a) we receive your course registration form, and (b) we complete the review and accept the course for registration.
  • The login is easy! Your login ID is your four-letter CE Provider code.
  • The form will calculate the fee for you if you are registering a cooperative offering with an organization that is not an ASHA approved CE Provider.
  • The form provides a list of what courses you have elected to "Save and Edit Later" and courses you have "Submitted."

What are required fields?

Required fields are marked in red, and they must be completed before you can move from one section to the next within the form. You must complete all required fields before submitting your course registration.

What is my member number?

Your member number is your four-letter provider code, for example, ABCD.

Using the E-Form

How do I save the form?

Instructions to Save and Edit Later

  • Select the Save and Edit Later button to save a draft of your ASHA CE Electronic Course and Offering Registration Form.
  • Press the Print Confirmation button, which includes your login ID (in this case, your e-mail address) and a secure password that you will use to access your saved form and continue editing. The password is a unique code created by the form. You will always use the same code for your password. The password code does not change for every course you submit.
  • The login and password that you use for the secure member portion of the ASHA website will not work here.
  • Once you submit your electronic form, you will not be able to access your copy of the form again.
  • If you submit the form in error, please contact your CE Provider Manager.
  • Follow these steps to access a course that you saved but did not submit:
    • When you are on the "Welcome!" page, you will see a link on the left side of the screen. Select Editing CE Course Service Center.
    • You will see a list of courses that you have submitted and courses that you are working on.
    • Select the page icon under the column titled "Edit" to resume work on the form.
    • You cannot access forms with a status of "submitted."
    • You can delete forms that you are working on by selecting the trash icon in the column titled "Withdraw."

Instructions to Save the Form Prior to Submission

  • If you do not have all required information to complete the form and you want to "save as you go," you can type an "X" in any required field (marked with the red asterisk).
  • When you get to the bottom of the form, select Save and Edit Later.
  • Then, press the Print Confirmation button, which includes a secure login and password that you will use to access your saved form and continue editing later.
  • When you have all of the required information, you can access the draft version of your form and fill in the correct information.
  • Fields not marked as required can be left blank.

How do I print a copy for my records?

The form will give you the opportunity to print a confirmation copy for your records; it also includes a print preview funciton that allows you to review the form prior to submitting it.

Can I edit the form after I submit it for review?

No. You cannot edit the form after you have submitted it. If you have submitted the form in error, please contact your CE Provider Manager.

What are the word counts in the Course Title and Course Description?

There is a limit of 80 characters for course titles and a limit of 400 characters for course descriptions. The electronic form will not let you enter more than the allowable number of characters, and as you are typing, the form displays the number of characters you have left.

How do I calculate the CEU amount?

Sixty minutes equals 0.1 CEU, and 90 minutes equals 0.15, etc. Learn more about CEUs in half-hour increments. Enter the CEU amount as a decimal, for example, 1.2 (12 hours), 0.3 (3 hours), 1.0 (10 hours), 0.15 (one and one-half hours).

How many additional offerings can I enter on the electronic form?

You can enter up to 12 offerings. For more than 12 offerings, use the Additional Offerings Form.

Uploading the Required Attachments

What documents will I need to upload to the Electronic Course and Offering Registration form?

For all courses you will upload the three required attachments: learning outcomes, time-ordered agenda, and the primary promotion materials that include the ASHA CE Provider Brand Block and CEU sentence.

The electronic form allows you to upload each of these documents individually, or, if you have a brochure that includes all three documents, the electronic form allows you to upload one document that contains all three required attachments.

For Individual Learning Experience (self-study), you will need to upload the following documents:

  • Self Study Product Information Form
  • Peer Reviewer Comments Form
  • Peer Reviewer Credentials Form
  • CEA Response to Peer Reviewer Comments Form
  • Pilot Study Forms (if needed)

Where do I find the forms I need to upload for Individual Learning Experiences (self-study)?

The forms you need are all on the ASHA CE Providers webpage. You will need to download them to your computer and complete them prior to entering the course registration information into the electronic form. You can then browse your computer for these files and upload them directly into the electronic form.

Can I scan all my Individual Learning Experience forms into one document?

No. You should scan your peer review forms into separate documents as follows:

  • Self Study Product Information Form
  • Peer Reviewer Comments Form
  • Peer Reviewer Credentials Form
  • CEA Response to Peer Reviewer Comments Form
  • Pilot Study Forms

Can I send my peer review forms and/or pilot study forms in the mail?

How do I prepare the documents I need to upload to the electronic form?

  • Prior to entering information into the electronic form, create the required attachments in either a Word file (.doc) or Adobe file (.pdf) and save them to your computer. Do not attach (.docx) files.
  • For Individual Learning Experiences (Self-Study), first download the forms from the ASHA CE website.
  • Complete and sign the forms.
  • Scan the forms into a Word (.doc) or Adobe (.pdf) document.

What file formats can I use for the documents I need to upload to the form?

The two acceptable formats are Word (.doc) or Adobe (.pdf).

What format should the file names be in for the documents I upload?

Do not use spaces or symbols in the file names (e.g., #, &, %, $, @). ASHA CE cannot open documents that have names with spaces or symbols. You may, however, use an underscore where you would otherwise leave a space.

Here are some examples of acceptable file formatsand document names:

  • 3_Learning_Outcomes.doc
  • PromoMaterialsAZBA1122.doc
  • timeorderedagenda.pdf

Do not use the following in your document file names:

  • Symbols such as: #, &, %, $,@
  • Spaces

Why can't I open the file that I just uploaded to the form?

Because you will not be able to select the link that appears in the "upload" field to review and ensure you that have uploaded the correct document, you will need to select Save and Edit Later when you reach the end of the form. When you access the course to "Edit Later," select the Edit CEU Registration Information icon to go back to the "Course and Offering Information" section of the form.

When you scroll to the "file upload" fields, you will see the name of the file above the upload field. Select the link to see the document that you uploaded. If it is incorrect, you can delete it and upload the correct document.

Cooperative Offerings and Payments

Why does the form ask if the cooperative party is an ASHA CE provider?

The electronic form will calculate the cooperative offering fee based on the number of cooperative offerings that you enter into the form and whether the cooperative party is an ASHA Approved CE Provider.

No cooperative offering fees are assessed when the cooperative party is an ASHA Approved CE Provider.

How do I know what the fees are for cooperative offerings?

You can go to the ASHA website for a list of all ASHA CE fees, however, the electronic form will calculate the fee for you.

The form gave me the incorrect amount for the cooperative offering fee, what can I do to correct it?

You may get an incorrect calculation if you changed your answer to either of the following questions:

  • VII.G. Is the Cooperative Party an ASHA Approved CE Provider?
    This is a "yes" or "no" answer. If you selected "yes" and then went back and changed it to "no", the form may calculate the fee incorrectly.
  • VII.H. How many of the offerings you are registering are Cooperative Offerings?
    This question asks you to select from a series of numbers (1–12) that represent the number of cooperative offerings that you are registering. If you select one number but then go back and change it to a different number, the form may calculate the fee incorrectly.

To correct the miscalculation, select the Clear button on the page entitled "Cooperative Offering Fees." This will clear the answers to the question that you need to change in section VII.G or VII.H. Then go back to question VII.G or VII.H and enter the correct answer. The form will recalculate the fee amount. If the amount is still not correct, please contact your Provider Manager.

What credit card payment does the form accept?

ASHA accepts Visa, Mastercard, and Discover, but not American Express.

How do I submit a cooperative offering fee with a check?

Print out a confirmation copy of the electronic Course and Offering Registration Form and include that with your check. Make the check payable to "American Speech-Language-Hearing Association." Mail the check and form to the following address:
