To assist programs in preparing their online submissions to CAA using the HES, data collection worksheets are available online (in Microsoft Word). Please note: Information collected on these worksheets for faculty, courses, and clinical sites on these worksheets will not be automatically uploaded to the HES. Programs will need to enter and confirm the CAA report data in the HES during the submission period.
